recently, in what quantities and what ministry you have had to overcome any
addictions:

If so, please describe the treatment received, dates, any lingering difficulties.

If so, please explain the circumstances briefly, the time and length of involvement
and what ministry you have had to overcome them:
1. The occult?
2. Cult or sect (new age, eastern mysticism, naturalistic philosophies
Mormonism, Jehovah’s Witnesses, etc.)
3. Heterosexual sexual interaction. How long have you been clean from this activity?
4. Pornography. If so, when was last time you were involved with it? Describe briefly
your struggle with it. Is it an addiction or habit?
5. Homosexual sin, if so how recent? Were there homosexual activities? Long-term
homosexual relationships? Have you ever had counseling about this issue?
d. Do you have a history of abuse, verbal, physical or sexual? Briefly summarize
this.
e. Have you ever been in prison? Convicted of a felony? Please summarize.

You may email your application answers to office@expression58.org, mail them to the
address listed below.
Applications will be followed up with a phone or personal interview
Applicants will be notified within 7 days of phone interview
$500 of the internship fee is due within two weeks of acceptance. The
other $3000 fee is due upon entrance of the program (For a total of $3500).
Checks may be made out to “Expression58” with internship application fee”
in the memo.
If possible, include a short video introducing yourself and talking about your life. It
helps us get to know you. Be creative and share things about your life right now.

Missions Experience
- Have you ever been on a missions trip before? Please tell us where, when, the nature of the outreach, what organization you went with, how long the trip was, and what your role was.
